He had a chance at 2 outs before that, but the ball he threw away, wasn't routine, but was clearly concerned about being taken out and botched the throw. Viciedo was playing shallow which he probably should do against Revere. But he didn't take a good route to try to catch the ball so it went over his head. The runner (Doumit) wouldn't have been at 2B, though, for Valencia. So the 4th and tying run would have been stranded. So you have the botched ball (not an error, but he regularly makes it) against Span, the ball thrown away (could have been an out and only 4-2), then Doumit advancing to 2nd and scoring on the Valencia hit.
